Effective January 1, 2008, WCCCA requires a deposit of $22.00 (check or money order) to cover the cost of producing information being requested. $22.00 pays for 15 minutes of work; should we determine your request will take more time you will be notified. Work on your request will begin when the full deposit is received.

Fill-in general information, making sure to include date, your name and contact information, the type of information you are requesting and the incident information to the best of your knowledge.
If you are requesting a medical call, please include a signed medical release.
Mail completed form to P.O. Box 6375, Beaverton, OR 97007 with your deposit. Requests will NOT be processed until the full deposit is received.
The standard processing time is usually 3 days to 3 weeks, depending on the details of the information requested, its accessibility and whether or not the incident is a criminal case as well as if your request requires authorization from the Washington County DA. There are no guarantees for processing time so please plan accordingly. For questions or concerns call 503-690-4911 x214; messages are checked periodically. Cancellations made by phone may be subject to processing fees.

Payment is required whether the information requested is collected or not.
The WCCCA Information Request Form is for information related to a Washington County 9-1-1 Emergency or Non-Emergency phone call. Other information related to an incident may be solicited from the responding police, fire or medical agencies.
